The U.S. Department of Education, Office of Special Education Programs requires states, to survey the parents of students identified as needing special education services. The survey asks parents with a child receiving special education services if schools facilitated parent involvement as a means of improving services and results for children.

Parent(s) or guardian(s) of a student with a disability who is receiving special education services will receive a survey, a self-addressed business reply envelope, and a cover letter from the Superintendent of Education at the annual Individualized Education Program meeting.
